Jerusalem and the Templars

Hello you all,
I've read that although the Templars absolutely disappeared from history the
Hospitalarios survive to our times as the Knights of Malta.
What was the original etymological meaning of "Hospitalarios"?  When did the
term become almost exclusively associated with medical facilities?
Does anyone know where the Knights of Malta maintain a presence in Jerusalem?
And lastly, does any of you know of a good book on Saladin?
