Something of a fallout from the standards process in a large document.
Often sections are not revisited for years and different editors (and
suggestors of text) have different styles.  Pulling these sort of things
into line takes a lot of time (reading, reviewing, proposals, votes for
change, etc.) that perhaps is not always worthwhile.  It's very different
from writing a book with only one author.
